Applications will only be processed if:

They are complete, accurate and legible

All registration fees must accompany the application. Checks and/or money orders should be made payable to Touched By Fire Angels, Inc. No applications will be complete until checks are cleared.

Team Registration - All team members must submit an individual Application form and a signed Waiver of Liability Form.
- Each team must name a Team Captain for any sport / event involving three (3) or more participants.
- The Team Captain must submit a Tearm Roster
- Team Members must list the last name and first initial of your Team Captain on your entry form.
- All non-playing coaches and managers must submit a signed Waiver of Liability Form.
- All teams must list complete names of each team member and their department.
